Primary effects
- Euphoria, disorientation, and a sense of disconnection can occur.
- Causes numbness, particularly of the extremities, and a warm, fuzzy inebriation in the headspace.
- Music appreciation is enhanced, and anxieties may dissipate.
Peak character
- A distant, hazy dissociation, feeling as if weirdness is experienced through a filter.
- A drifty aura with sensory inputs hitting as waves of sound and vision; not intensely euphoric but a comfortable ride.
- Lethargy and reduced motivation to engage, making social interaction difficult to maintain discreetly.
Negatives / side effects
- Higher doses can lead to anxiety, delusions, nausea, memory suppression, and in extreme cases, blackout, delirium, psychosis, or death.
- It is crucial to identify DXM products without other active ingredients, as some additives are dangerous at high doses.
Aftereffects / comedown
- Effects slowly wind down, with a comfortable good night's sleep.
- A mild afterglow is possible the next morning, contributing to a refreshed feeling.
- Some individuals may experience a mild downturn for a few days following the experience.

Adulterant detection: Mecke going blue/black is highly characteristic for DXM. The primary risk is DXM cough syrup products containing antihistamines (chlorphenamine) — these are dangerous in large doses. Fentanyl in illicit DXM powder has been reported.
Use pharmaceutical cough syrup with DXM as sole active ingredient — test the label. For illicit DXM powder: Mecke is the primary confirmatory test; add fentanyl strip.
